All forms of payment are accepted- cash, check, credit or debit card.
Co-pay charges are usually displayed on your insurance card.
Many insurances require patients to pay an annual deductible amount before insurance pays for services. If your insurance reports that you are responsible for payment of a deductible, the practice will notify you by email.

Please bring identification and insurance card. If your insurance requires a primary care provider, please call to name Maxwell Hogue NP as your PCP.
Cancellations and No-Shows
Please cancel appointments 24 hours in advance or you may be charged.

TDI Temporary Disability Insurance
TDI no longer faxes or mails the medical certification forms (TDI-3 and TDI-3C) to Healthcare Providers.
TDI mails the required medical certification form (TDI-3 or TDI-3C) to the applicant’s home address. It’s the applicant’s responsibility to provide the form to the appropriate treating Qualified Healthcare Provider (physician). You may scan the form. Then fax to me at (401) 340-1656 or email [email protected]
